Aim of Facial Feminization Surgery

Dr. Kamol’s principle of Facial Feminization Surgery  (FFS) is based on aesthetic fundamentals as follows:

  1. To balance the facial proportions of the upper, middle, and lower face ratio, divide into five parts from left to right to create an unmistakably feminine face.
  2. The balance of facial components is related to the eyes, eyebrows, nose, cheeks, and lips, making the face female.
  3. Facial frame, including hairline, side cheeks, jaws, and chin, to remove male features and, where needed, create feminine ones.
  4. The balance of facial bone and soft tissues to be more feminine.

The 8 Points of Facial Feminization

FFS procedures can be categorized based on the three main areas of the face:

  • Upper Face: Forehead and eyebrows.
  • Midface: Eyes, nose, and cheekbones.
  • Lower Face: Lips, jaw, chin, and Adam’s apple.

1. Forehead Feminization

The forehead takes up most of the upper face, and people subconsciously use it more than any other feature to identify gender. There are three main differences between male and female foreheads:

2. Eyebrow Feminization

There are two significant differences between male and female eyebrows:

  • Male eyebrows are thicker and straighter, whereas female eyebrows are thinner and often arched.
  • Male eyebrows are lower and closer to the orbital eye rim, while female eyebrows are higher.

3. Nose Feminization

The nose is a very visible gender marker and sits at the centre of the face, making it one of the most important features to refine. Male noses tend to be broader and longer with a prominent bridge; female noses are narrower, shorter, and gently curved. We offer three approaches to rhinoplasty:

  • Reduction rhinoplasty: usually recommended for Caucasian patients, to reduce a hump, side bones, and the nasal tip, and to refine the nostrils. (see more)
  • Augmentation rhinoplasty: usually recommended for Asian patients, enhancing the nasal ridge and tip with silicone, e-PTFE, ear cartilage, or rib cartilage. (see more)
  • Combined technique: for patients who need both ridge reduction and tip augmentation to achieve the most balanced result. (see more)

4. Cheekbones Feminization

Female cheekbones are typically higher, more curved, and prominent, making the face look fuller, while male cheekbones are lower, flatter, and more rectangular. We offer three procedures: cheek augmentation, cheekbone shaving, and cheekbone (zygoma) reduction.

5. Eyes Feminization

The eyes help signal gender. Male eyes tend to look deeper-set and narrower, while female eyes look larger and brighter. 

We offer:

  • Cat eye (almond eye) surgery, which widens the eyes by opening the eyelids and lifting their outer edge upward; the eyebrows may also be raised to create more distance between the eyes and brows.
  • Foxy Eye surgery, in which the outer corners of the eyes are gently lifted and secured to the orbital bone for a more attractive upward angle. Excess upper eyelid skin can also be removed to create a refined double-eyelid contour. 

The cat-eye procedure is performed under local anesthesia or IV sedation, while the Foxy eye procedure is performed under General anesthesia and IV sedation, and takes about 2–3 hours. The outer canthus is moved upward, and excess eyelid skin is removed. Stitches are removed five days after surgery. Patients can open their eyes normally afterwards and resume light activity within a week. (see more)

6. Lips Feminization

Lips are one of the highlights of feminine beauty. Male lips are naturally thinner and straighter, while female lips are more curved, plumper, and fuller.

7. Adam’s Apple reduction (Tracheal Shaving)

The Adam’s apple is a uniquely male feature, a clear marker people use to gender one another. To achieve a feminine appearance, Adam’s apple needs to be removed.

8. Chin and Jaw Feminization

The chin and jaw are strong gender markers. 

  • Male chin has wide, broad and more protruding chins, while female chin have gently pointed and curved chin with less protruding features
  • Male jaws have broad and large jaws with more prominent muscles and saliva glands, while female jaws have narrow jaws with invisible saliva glands.

Duration Time of the Surgery

The duration of surgery depends entirely on the procedures performed. The average time for FFS is around 3-6 hours, depending on the procedures performing. Afterwards, you will spend a few hours in the recovery room, as is standard following any general anesthetic, then move to a private room where you will gradually wake up. Pain is generally well tolerated, and you will be given strong painkillers in the first few days to keep you as comfortable as possible. You will also have an alarm button by your bed lets you call the nurses if you feel pain between standard doses.

Preparation for FFS

  • Medication review: stop aspirin, anti-inflammatory drugs, or herbal supplements that affect blood clotting at least 2 weeks before surgery, or as directed.
  • Smoking cessation: stop smoking and alcohol for at least 2 weeks before surgery to promote proper wound healing.
  • Hormone management: stop hormone therapy for 1–2 weeks before surgery, as advised by your doctor.
  • Choose an expert: consult a surgeon specializing in FFS, as these procedures are complex and require extensive experience.
  • Complete health assessments: blood tests, chest X-ray, electrolytes, HIV test, and a facial CT scan; needed in complicated and revision cases, so the surgeon can plan the most suitable techniques and prepare for anesthesia.
  • Fasting: if you are having general anesthesia, abstain from food and water for at least 6 hours before the procedure.

Recovery

Recovery depends very much on which procedures are performed; your doctor will discuss the expected timeline during your consultation. In most cases you will need around three weeks off work. As with most surgery, you will look “normal” fairly quickly, and results continue to improve over several months as tissues and swelling settle.

Post-operative Care

  • Elevate your head: sleep with your head raised on two pillows or in a semi-reclined position to minimize swelling.
  • Maintain oral hygiene: if your surgery involved the mouth, rinse frequently with mouthwash and eat a liquid or soft diet while stitches are new.
  • Wear a facial bandage: use the provided bandage for 2–3 weeks to reduce swelling and support healing.
  • Keep wounds clean: clean the wound daily to prevent infection. You can wash your hair 2 days after surgery.
  • Stitch removal: stitches are typically removed 7 days after surgery.
  • Return to work: most patients return to work about 2 weeks after surgery; wounds should fully heal within 4–8 weeks.
  • Avoid strenuous activity: refrain from strenuous exercise for at least 1 month.
  • Attend follow-up appointments: see your doctor for scheduled check-ups so healing can be monitored.

Risks and Complications

While the procedure is generally safe, it’s important to be aware of potential risks, which may include:

Travel is another consideration. Long-haul flights after surgery can increase clot risk, which is why patients are advised to remain in Bangkok for the recommended recovery period rather than flying home early.

It depends on your circumstances. Travelling to Bangkok may make sense if cost is a barrier, waiting lists are long, you want multiple procedures performed together, or FFS is not well established where you live.

Staying local may be better if you have medical conditions requiring ongoing supervision, cannot take three weeks away, or have insurance that only covers treatment domestically.

Plan for around three weeks. This allows time for surgery, initial recovery, stitch removal at around seven days, and follow-up appointments before flying home.
