Facial Masculinization Surgery (FMS) in Bangkok, Thailand

Sculpts facial features through procedures such as jaw, chin, and brow enhancement to create stronger, traditionally masculine contours.

Facial masculinization surgery (FMS) is a group of procedures that reshape the facial bones and soft tissue to create a more masculine appearance. It’s most commonly chosen by trans men and transmasculine people to align their face with their gender identity.

Typically male and female facial features differ in many subtle ways, so FMS works best when the whole face is planned together. It’s performed by plastic surgeons with a deep understanding of both male and female facial structure. In some cases, a maxillofacial surgeon joins the team to reshape the jaws.

The 6 Areas of FMS

1. Masculine Forehead

Male skulls typically show more prominent brow bossing over the frontal sinuses, giving a stronger, flatter forehead with a defined ridge above the eyes.

Two approaches are used, often together:

  • Forehead augmentation builds up the brow ridge and forehead to a more masculine projection.
  • Forehead lengthening raises the hairline, increasing the distance between the brows and the hairline — a typically male proportion. This may be combined with hair transplantation to create a squarer, higher hairline with defined temporal recession.

2. Masculine Cheekbones

Male cheekbones typically read as flatter and more angular than female cheekbones, sitting lower with less roundness. Depending on your existing structure, the right approach may be cheekbone augmentation to add angular definition, or cheekbone reduction where rounded fullness is softening the midface. Your surgeon will assess which applies to you.

3. Masculine Nose

The male nose tends to be longer and broader, with a straighter bridge and less upward tip rotation. Masculinization is usually achieved through augmentation rhinoplasty, increasing the projection and width of the nasal bridge, sometimes with adjustment to the tip angle. 4. Masculine Chin

A male chin tends to be larger, longer, wider and squarer. The chin is usually reshaped with a chin implant combined with bone reshaping (genioplasty) to create a broad, square shape.

5. Masculine Jaws

A male jaw has a wider lower jaw (mandible) and a more prominent jaw angle. Jaw augmentation and contouring add width and definition at the angles, balanced against the overall width of the face. The jaw is often reshaped together with the chin, so the lower face looks harmonious from every angle.

6. Adam's Apple Enhancement (Thyroid Cartilage Augmentation)

The Adam’s apple, the thyroid cartilage at the front of the voice box, is one of the most noticeable male features. It can be enhanced with an implant such as ePTFE or silicone for a modest increase, or with rib cartilage, which allows a greater, more natural projection. The procedure can be performed under local or general anesthesia.

A Good Candidate

  • Trans men, transmasculine, and non-binary people seeking a more masculine facial appearance
  • In good general health
  • A non-smoker, or willing to stop before and after surgery
  • Holding realistic expectations about what surgery can achieve
  • Prepared for a recovery measured in weeks

Many patients have been on testosterone for some time before FMS, since hormone therapy itself changes facial fat distribution and can affect surgical planning. Your surgeon will discuss timing at consultation, along with any documentation requirements that apply to your situation.

Preparation

  1. Consult a plastic surgeon highly experienced in facial gender-affirming surgery. Facial bone surgery is complex, carries higher risks, and several procedures need to be planned together.
  2. Have a physical examination, blood tests, chest X-ray, electrolyte levels and an HIV test.
  3. Have a facial CT scan so your surgeon can plan the technique and prepare for general anesthesia.
  4. You may be asked to pause hormone therapy for 1–2 weeks before surgery.
  5. Stop aspirin, anti-inflammatory drugs, vitamins and herbal supplements that affect blood clotting for at least 2 weeks, or as your doctor advises.
  6. Stop smoking and drinking alcohol for at least 2 weeks to support healing.
  7. If you’re having general anesthesia, don’t eat or drink for 6 hours beforehand.

The FMS Process

  1. Consultation: your surgeon assesses your face and discusses your goals.
  2. CT scan and confirmation: after the CT scan, you meet the surgeon again to confirm which procedures you’ll have.
  3. Pre-admission tests: carried out a few days before surgery.
  4. Surgery: performed under general anesthesia. Operating time depends on the number of procedures.

Post-operative Care

Postoperative care varies for each procedure, but the basics are as follows.

  • Have a liquid or soft food.
  • Always keep your mouth clean with mouthwash.
  • Avoid aggressive activities for a month after surgery
  • Avoid smoking
  • Don’t over-exercise within one month.
  • Take a painkiller to reduce pain.
  • Patients can resume normal activities two weeks after surgery. Wounds should ultimately be healthy within 4-8 weeks.
  • Keep any follow-up appointments with your surgeon to check the healing process and ask any questions that may concern you

Recovery

  • First week: swelling and bruising are most noticeable.
  • About 2 weeks: most patients return to work, although some swelling remains.
  • About 1 month: you can return to exercise.
  • 4–8 weeks: incisions are fully healed.
  • 3–6 months: the new facial shape settles into its final form.

Risks and Complications

  • Infection of the incision or an implant
  • Numbness around the surgical area
  • Swelling (edema)
  • Hematoma (blood collection)
  • Bleeding
  • Pain, managed with medication and cold compresses
  • Bruising
  • Nerve injury, which may be temporary or, rarely, permanent
  • Facial asymmetry
  • Implant problems, such as shifting or rejection, which may require removal
  • Poor wound healing
  • Tissue necrosis
  • Scarring

Before and After for Facial Masculinization

Why Many Trans Men Choose FMS

For many trans men, facial features traditionally seen as feminine can be a source of gender dysphoria. FMS can ease that distress by bringing the face into line with one’s identity. Because people often read gender from the face, a more masculine appearance can also reduce misgendering, unwanted attention and discrimination in everyday life.

FMS is one part of transition, not a requirement. Some trans men are comfortable with their facial features, or choose to focus on other steps first. For those who do choose FMS, it can make a profound difference to how they feel in their own body and in the world.

Frequently Asked Questions (FAQs)

Facial Masculinization Surgery (FMS) is a set of surgical procedures designed to alter your facial features, creating a more masculine appearance, if so desired.

This is a common choice for transgender men and other individuals who are seeking to align their facial features with their gender identity.

FMS can involve a variety of cosmetic procedures tailored to address specific areas of the face, such as the forehead, jawline, chin, nose, and Adam’s apple.

Facial masculinization surgery encompasses a range of procedures, including:

  • Forehead contouring: Reshaping the forehead to create a more prominent brow ridge and flatter forehead.
  • Jaw contouring: Augmenting the jawline with implants or bone grafts to create a stronger, more defined jaw.
  • Chin augmentation: Enhancing the chin with implants or bone grafts to create a more prominent and masculine chin.
  • Rhinoplasty: Reshaping the nose to create a more masculine appearance.
  • Cheek augmentation: Adding volume to the cheeks with implants or fat grafts to create a more defined and angular appearance.
  • Thyroid cartilage enhancement (Adam’s apple surgery): Enhancing the prominence of the Adam’s apple.
  • Lip lift: Shortening the distance between the nose and upper lip, creating a fuller upper lip.

Note:
The specific procedures included in your facial masculinization process will depend on your individual needs and desired aesthetic outcome.

You might be a good candidate if you:

  • Are a transgender man or gender-diverse individual seeking to have a more masculine facial appearance.
  • You have cave completed hormone therapy (if applicable).
  • Are a non-smoker.
  • You are in good overall health.
  • Have realistic expectations about the outcome.

First, we recommend scheduling your consultation with one of our qualified plastic surgeons specialising in the procedure at Kamol Cosmetic Hospital. At that point, they will evaluate your facial features, discuss your long-term goals, and determine which procedures are most suitable.

The procedure comes with a number of attractive benefits, including:

  • More masculine facial features: Achieve facial features that align with your gender identity.
  • Increased self-confidence: Feel more comfortable and confident in your appearance.
  • Enhanced social interactions: Experience greater social acceptance and ease in social situations.
  • Improved quality of life: Feel more authentic and comfortable in your own skin.

Often, yes. Testosterone gradually changes the face over the first one to two years, redistributing facial fat, thickening the skin and, for many, bringing in facial hair. Waiting lets your surgeon see how your face has changed, so surgery only addresses what hormones haven’t. A beard can also soften how much jaw or chin change you need.

Sometimes. Several facial procedures are commonly done in one operation. Combining FMS with top surgery or a hysterectomy is possible in some cases, but it lengthens the operation, so it is often safer to stage them. Your surgeon will plan the safest sequence for you.

Bone reshaping and implants give long-lasting results. Your face will continue to age naturally around them, and in rare cases an implant may shift or need adjusting. Rib cartilage used for the Adam’s apple becomes a permanent part of your own tissue.

Most incisions are hidden. Jaw, chin and cheekbone procedures are usually performed through incisions inside the mouth. Forehead procedures use incisions in or along the hairline, and nose incisions are small or inside the nostrils. Adam’s apple enhancement uses a small incision placed in a natural neck crease.

Plan to stay in Bangkok for about 10–14 days. This allows for stitch removal at 7 days and follow-up checks before you fly home. If several bone or implant procedures are combined, allow a little longer. Thailand is one of the world’s most experienced destinations for gender-affirming surgery, and Kamol Trip can arrange accommodation close to the hospital.
