Revision Vaginoplasty / SRS / Gender Affirming in Bangkok, Thailand

Corrects or enhances results of previous vaginoplasty, improving appearance, function, and comfort for better long-term surgical outcomes.

Revision vaginoplasty (secondary vaginoplasty) is surgery to improve the result of a previous gender reassignment surgery. It can restore lost depth, correct narrowing, improve the appearance of the vulva, or replace an older vaginal lining with a newer technique.

Revision is more complex than first-time surgery, because tissue may be scarred or limited. Kamol’s team includes plastic surgeons, urologists, colorectal surgeons, gynecologists and anesthesiologists, who plan each revision around the tissue that remains.

Common Reasons for Revision

Functional problems

The most common technique worldwide, penile inversion, relies on regular dilation. Without it, the vaginal canal can narrow until intercourse is no longer possible. In other cases, depth was never adequate after the first surgery.

If narrowing is caught early, a structured dilation program may help. Once the canal has closed or become very shallow, surgery is usually needed to rebuild the lining, using peritoneum (PPV), sigmoid colon or skin grafts.

Aesthetic concerns

The appearance of the vulva matters to many trans women’s confidence. Revision can improve:

  • The shape and proportion of the labia minora and majora
  • The position of the urethra and vaginal opening
  • The appearance of the clitoris and clitoral hood
  • Visible scars

With limited skin and tissue, there are limits to what can be achieved. Your surgeon will explain realistic goals at consultation.

Upgrading to a newer technique

Some women with a good result from penile inversion choose to “upgrade” to PPV or colon vaginoplasty for natural lubrication, greater elasticity and less long-term dilation. Aesthetic changes can often be made in the same operation.

Revision Techniques at Kamol Hospital

1. Revision with Peritoneum (PPV)

The peritoneum is the thin, moist membrane that lines the inside of the abdomen. It’s elastic and produces natural lubrication. In revision PPV, peritoneum is harvested, usually laparoscopically, and pulled down to line the vaginal canal, combined with the existing penile skin at the entrance.

Using peritoneum to create a vaginal canal isn’t new: the Davydov technique has been used for more than 45 years for people born without a vagina, such as those with Mayer-Rokitansky-Küster-Hauser (MRKH) syndrome. During surgery, one surgeon prepares the peritoneum while Dr. Kamol reconstructs the vulva, and the two are then joined. See PPV.

2. Revision with Sigmoid Colon

The advantages of using the Sigmoid Colon are that the vagina will have a natural lubricant, and dilation for life is not usually needed. This method offers the most profound vaginal depth at 7-9 inches. Contrary to widespread rumors, the vagina doesn’t have a bad odor. The sigmoid colon is vital and rarely shrinks. At Kamol Cosmetic Hospital, there are 2 options for Sigmoid Colon SRS:

  • Open Technique: The sigmoid colon with open technique leaves a 5 cm scar on the bikini line. The average length of the sigmoid colon segment is approximately 7 inches. The operation time is approximately 6 hours.
  • Laparoscopic Technique (Keyhole surgery-No Scars): The sigmoid colon is harvested through 4 small incisions using the laparoscopic technique. There are 3 tiny freckle-like scars on the abdomen. The sigmoid colon segment is closed at the upper end and pulled through the neo-vaginal canal and anastomosis (re-connected) to the neovaginal opening.

3. Aesthetic Revision

The result of revision vaginoplasty depends on the previous surgery and the severity of damage that has been done. Our team includes plastic surgeons, urologists, colorectal surgeons, gynecologists, anesthesiologists, etc. They will create and design a new vagina with the limited quantity of skin and tissue available. Revision vaginoplasty is an art form of plastic surgery, as you can see from our before and after gallery.

Who Is a Good Candidate?

  • Trans women who have had a previous vaginoplasty, at Kamol or elsewhere
  • Unhappy with depth, width, function or appearance
  • At least 6–12 months since the previous surgery, so tissue has settled
  • In good general health, and within the BMI limits for the chosen technique (see SRS)
  • Non-smokers, or willing to stop
  • Realistic expectations, given the tissue available

Assessment and Preparation

  1. Share your history: if possible, bring operative notes from your previous surgery, recent photos, and a note of your current depth with a dilator.
  2. Examination: your surgeon will examine you and recommend the best technique.
  3. Health checks: physical examination, chest X-ray and blood tests. If you’re over 40, an EKG and cardiac stress test are also needed.
  4. Hormones: stop hormone therapy for 1–2 weeks before surgery, as advised.
  5. Medication: stop aspirin, anti-inflammatory drugs and supplements that affect blood clotting at least 2 weeks before.
  6. Smoking: stop for at least 2 weeks before and after surgery.
  7. Bowel preparation: for colon vaginoplasty, an enema is given the day before surgery.

Recovery

Recovery is similar to first-time SRS using the same technique:

  • Hospital stay: about 4–7 nights, depending on the technique.
  • Diet after PPV or colon surgery: nothing by mouth until bowel function returns, then a gradual return to soft food.
  • Dilation: begins after the stitches are removed, at least twice a day.
  • Follow-up: weekly appointments during the first month.
  • Sex: avoid sexual intercourse for at least 2 months.

Risks and Complications

Revision surgery carries the same risks as first-time vaginoplasty, and some are higher because of scar tissue:

  • Bleeding, infection and delayed wound healing
  • Narrowing of the vagina or urethra
  • Recto-vaginal fistula (a rare abnormal connection between the rectum and vagina)
  • Bowel-related complications after colon or PPV surgery
  • Blood clots in the legs
  • Reduced sensation
  • Asymmetry or dissatisfaction with appearance
  • The need for further surgery
  • Risks from anesthesia

Revision Vaginoplasty Results

The result depends on the previous surgery and how much healthy tissue remains. Our multidisciplinary team designs each revision individually to achieve the best possible appearance and function with the tissue available.

Frequently Asked Questions (FAQs)

Revision vaginoplasty (also called secondary vaginoplasty) corrects or improves the results of an earlier vaginoplasty — addressing problems with depth, function, or appearance.

Anyone who has had a previous vaginoplasty and is unhappy with the depth, function, or aesthetic outcome, including cases caused by canal narrowing, scarring, or an inexperienced original surgeon.

Revision cases usually involve limited remaining skin and tissue, so the surgical plan is adapted to the individual — often using a peritoneal or sigmoid colon graft rather than reworking the original tissue alone.

Recovery time depends on the technique used and the complexity of the revision. Your surgical team will provide a personalized recovery and follow-up plan, including guidance on dilation if applicable, before you leave Bangkok.

Sometimes. If narrowing is caught early, a structured dilation program under medical supervision may restore some depth. If the canal has closed or become very shallow, dilation alone usually isn’t enough, and surgery is needed to rebuild the lining. Your surgeon will examine you and advise.

Yes, at least at first. Dilation helps the new lining heal at the right width and depth. With PPV or colon linings, long-term dilation needs are generally lower than with skin-lined techniques, but your surgeon will give you a personal schedule.

Our team has more than 30 years of experience across every major revision technique, including two of the newer, less widely available approaches — peritoneal pull-through and sigmoid colon vaginoplasty.

Plan to stay in Bangkok for about 3–4 weeks. This covers your hospital stay, the start of dilation and weekly follow-up appointments. Thailand has one of the world’s longest-established reputations for gender-affirming surgery, and Kamol Trip can arrange recovery accommodation close to the hospital.
