Facial contouring combines procedures on different parts of the face, such as the forehead, cheekbones, nose, jaw, chin, eyes, lips and skin, into one plan. Rather than treating each feature in isolation, your surgeon adjusts the underlying bone, the soft tissue that covers it, and the skin, so the whole face works together in proportion, balance and harmony.
Facial contouring can address features you were born with, asymmetry, or changes with age. It’s also a key part of gender-affirming care, through facial feminization and facial masculinization surgery.
How Surgeons Assess Facial Proportions
Your surgeon starts by assessing your face in two directions.
Vertical thirds: the face is divided into three roughly equal parts from hairline to chin:
- Upper third: from the hairline to the brows
- Middle third: from the brows to the base of the nose
- Lower third: from the base of the nose to the bottom of the chin
Horizontal fifths: from the left edge of the face to the right, the face is divided into five roughly equal parts, each about the width of one eye.
These guides help your surgeon decide which areas to change, and by how much, to create balance. The final plan also reflects your own goals and features, as well as your surgeon’s artistic judgment.

Corrective Jaw Surgery (Orthognathic Surgery)
When the upper and lower jaws are out of alignment, aesthetic changes need to go hand in hand with a healthy bite. Corrective jaw surgery includes:
- Le Fort I osteotomy: the upper jaw is cut and moved backward or forward to balance with the lower jaw.
- Bimaxillary osteotomy: the upper and lower jaws are repositioned in the same operation to balance the facial bones and improve the bite.
- Sagittal split osteotomy: the lower jaw is split on the inside and moved forward or backward.
- Sliding genioplasty: the chin bone is moved when the chin is short or recessed, but the teeth already bite together well.
Procedures 1–3 are often combined with orthodontic treatment.
Every procedure on the face affects bone, teeth, muscles and nerves. At Kamol, our aim is always both aesthetic and functional: the face should look balanced and keep working normally afterwards.
Facial Contouring for Gender Affirmation
For many trans women, feminizing the face is one of the most important steps in feeling comfortable and safe in daily life. Facial feminization surgery (FFS) combines several procedures, which may include:
- Forehead and brow: brow bone shaving, forehead reconstruction, hairline lowering, hair transplant and brow lift
- Eyes: upper and lower blepharoplasty
- Cheeks and midface: cheekbone reduction or augmentation, midface and perinasal augmentation, fat transfer, buccal fat removal and dimple creation
- Nose: rhinoplasty
- Lips: upper lip lift, lip augmentation or reduction (lip surgery)
- Jaw and chin: jaw reduction, chin reduction and mandibular contouring
- Skin and soft tissue: facelift, and non-surgical treatments such as laser and Botox for smoother skin
For trans men, facial masculinization surgery (FMS) uses similar techniques to create stronger, more angular contours.
Every surgeon’s style is different, so it’s worth reviewing the results of patients they’ve treated before. Every procedure carries risks, so take time to research and consult an experienced surgeon before deciding.
Ethnicity and Facial Features
Facial structure varies across ethnic backgrounds, so goals and techniques vary too. For example, many patients of Asian descent seek nasal bridge augmentation, while many patients of European or Middle Eastern descent seek reduction of a dorsal hump or refinement of a drooping nasal tip. A surgeon experienced with diverse patients will plan changes that suit your heritage rather than applying a single standard of beauty.
Who Is a Good Candidate?
- In good general health
- Non-smokers, or willing to stop before and after surgery
- Realistic expectations about the result
- Committed to following pre- and post-operative instructions
Preparing for facial surgery
- Consult with our experienced surgeon. Facial bone surgery is complex and carries higher risks, so it requires a plastic surgeon with extensive experience in facial structure. Several procedures may need to be planned together.
- Health checks and imaging. Blood tests, a chest X-ray and a facial CT scan help your surgeon plan the right technique. If general anesthesia is needed, you’ll consult an anesthesiologist.
- Hormones. If you take hormone therapy, you may be asked to stop for 1–2 weeks before surgery.
- Medication. Stop aspirin, anti-inflammatory drugs and herbal supplements that may increase bleeding and bruising for at least 2 weeks.
- Smoking. Stop smoking for at least 2 weeks to protect blood supply to healing tissue.
- Fasting. If you’re having general anesthesia, don’t eat or drink for at least 6 hours beforehand.
Post-operative Care
- Sleep with your head raised on two pillows, or semi-upright, to help swelling go down.
- Apply cold compresses for about 30 minutes at a time during the first few days to reduce swelling and discomfort.
- If you have incisions inside the mouth, rinse often with mouthwash to keep the mouth moist and clean.
- Wear the facial compression garment provided to support your face, reduce swelling and help shape the contours.
- Keep incisions clean every day, with no dried blood. You can wash your hair 2 days after surgery.
- Stitches are removed after 7 days. Most patients return to work after about 2 weeks.
- Avoid strenuous exercise for at least 1 month.
- Attend every follow-up appointment.

Frequently Asked Questions (FAQs)
What exactly is facial contouring?
Facial contouring is a surgical procedure, or series of procedures, designed to reshape and refine the features of your face. It can involve augmenting (adding to) or reducing (taking away from) bone and soft tissue to create a more balanced and harmonious appearance.
At Kamol Cosmetic Hospital, we offer a comprehensive range of facial contouring and rejuvenation procedures to help you achieve your aesthetic goals.
Which areas of the face can be addressed with facial contouring?
Facial contouring can address a wide range of areas, including:
- Cheekbones: Cheekbone reduction or augmentation can create more defined cheekbones and improve facial proportions.
- Jawline: Jaw reduction or jaw augmentation can create a more defined jawline and improve the balance between the upper and lower face.
- Chin: Chin augmentation can create a stronger, more prominent chin, while chin reduction can reduce the size of a prominent chin.
- Forehead: Forehead contouring can smooth out an uneven forehead or reduce a protruding brow bone.
- Nose: Rhinoplasty can reshape the nose to improve its size, shape, and proportion in relation to other facial features.
- Lips: Lip augmentation can create fuller lips, while lip reduction can reduce the size of overly large lips.
- Neck: A neck lift can address sagging skin and excess fat in the neck, creating a more youthful and defined profile.

Can several facial procedures be done in one operation?
Often, yes. For example, jaw reduction, chin reduction and cheekbone reduction are commonly combined to create a V-shaped face. Combining procedures means one anesthetic and one recovery period. If your plan involves many procedures, your surgeon may divide it into two or more stages for safety.
Is facial contouring surgery better than fillers?
They do different things. Fillers and other non-surgical treatments can add volume or smooth lines, but results are temporary and can’t reduce bone. Facial contouring surgery permanently changes the underlying bone structure, such as narrowing the jaw or cheekbones. Some patients use fillers first to see how a change might look before committing to surgery.
Will I still look like myself?
That’s the aim. Good facial contouring works with your existing features, so the result looks like a refined version of you rather than a different person. Your surgeon will discuss how much change you want and recommend an approach that keeps your face in natural proportion.
How long does recovery take?
Most patients return to work after about 2 weeks. Swelling is most noticeable in the first 1–2 weeks and gradually settles, but bone surgery can leave subtle swelling for several months. The final shape is usually visible after 3–6 months.
Where will the scars be?
Most facial contouring incisions are hidden. Jaw, chin and cheekbone procedures are usually performed through incisions inside the mouth. Forehead and hairline procedures use incisions along or behind the hairline, and eyelid scars sit in the natural crease. Your surgeon will explain the incision locations for your specific plan.
How long should I stay in Bangkok for facial contouring?
Plan to stay in Bangkok for about 10–14 days. This allows for stitch removal at 7 days and follow-up checks before you fly home. If you’re having several bone procedures, allow a little longer. Thailand is one of Asia’s leading destinations for facial surgery, and Kamol Trip can arrange accommodation close to the hospital.
