A high or broad forehead can leave the face feeling out of proportion, and for many people it’s a long-standing source of self-consciousness. Hairline lowering — also called forehead reduction or scalp advancement — moves the hairline forward to shorten the forehead and bring the upper third of the face into better balance.

Who Is a Good Candidate?

You may be suitable for hairline lowering if you:

  • Have a naturally high hairline or a tall forehead that you’d like to shorten
  • Have a hairline that has been stable for some time, with no ongoing recession
  • Have a reasonably flexible scalp, which allows it to be advanced forward
  • Are in good general health
  • Have realistic expectations and understand the risks of surgery

When It May Not Be Suitable

  • If your hairline is still receding, for example due to male-pattern hair loss. Lowering it now could leave the scar exposed if hair behind it thins later. A hair transplant, or treatment to stabilise the hair loss first, may be a better option.
  • If your scalp is very tight. The hairline may only move a small distance, so combining with a hair transplant is usually recommended.
  • If your brows sit low or droop. Hairline lowering doesn’t raise the brows. A brow lift may be more suitable.

Your surgeon will assess your scalp laxity, measure your forehead and discuss which option suits you at consultation.

Preparation

  • Consult with a plastic surgeon who is highly skilled and experienced in hairline lowering. You will discuss your surgical goals, medical condition, and treatments
  • Physical examination, including a blood test, chest X-ray, electrolytes level, HIV
  • Stop smoking and drinking alcohol at least one week before surgery
  • Avoid taking aspirin, anti-inflammatory drugs, and herbal supplements as they can increase bleeding and bruising for at least two weeks
  • Stop eating and drinking at least 6 hours before surgery

How Much Can the Hairline Be Lowered?

In a single operation, the hairline can usually be brought forward by about 1.5–2 cm, depending on how flexible your scalp is. If you’d like a larger change, or want to reshape the corners of the hairline, hairline lowering can be combined with a hair transplant.

Techniques

Hairline Lowering

The incision is made along the front of the hairline using a trichophytic technique. Rather than a straight line, it follows the natural irregular edge of the hairline, and the skin edge is bevelled so hair can grow through and in front of the scar. This makes the scar much harder to see once healed.

Hair Transplant

  • Follicular Unit Transplantation (FUT): A strip of scalp is taken from the back of the head, divided into individual grafts and placed along the new hairline.
  • Follicular Unit Extraction (FUE): Individual follicles are removed one by one with a fine instrument. Healing is faster, but the procedure takes longer.

The Procedure

  1. Anesthesia: Hairline lowering is performed under general anesthesia.
  2. Incision and advancement: The surgeon makes an incision along the hairline that extends into the temples within the hair-bearing scalp. The scalp is released and advanced forward, the excess forehead skin is removed, and the hairline is set in its new, lower position.
  3. Closure: The incision is closed in layers to minimise tension and scarring. Once the hair grows back, the scar is almost entirely hidden.

The operation takes around 2 hours.

Aftercare and Recovery

  • Most patients can wash their hair the day after surgery.
  • Keep your head elevated for 1–2 weeks to reduce swelling.
  • Take the medication provided to control pain and swelling.
  • Most patients return to desk work within a few days.
  • Stitches are removed after about 7 days.
  • Avoid strenuous exercise for 2–3 weeks, and protect the scar from direct sun while it heals.
  • In some cases, a hair transplant is recommended later to lower the hairline further or to camouflage the scar.
  • Follow all post-operative instructions and attend every follow-up appointment.

Swelling and tightness across the forehead settle over the first few weeks. The scar continues to fade over 6–12 months.

Risks and Complications

As with any surgery, there are risks. These may include:

  • Bleeding or a collection of blood under the skin (hematoma)
  • Infection
  • Visible scarring, including pigment changes in some people with darker skin
  • Wound separation along the incision
  • Temporary hair shedding near the incision (shock loss)
  • Numbness or altered sensation of the scalp, usually temporary
  • Facial nerve injury, which is rare
  • Reactions to anesthesia
  • Dissatisfaction with the result

The result of Hairline Lowering

Hairline lowering will pull down hairline around 1.5-2 centimeters. Make your forehead more narrow 1.5-2 cm. Your face will be more balanced with Hairline lowering and Hair transplant.

  • The maximum of pull-down is approximately 2 cm. Maybe not enough for some patients’ needs
  • Hairline lowering surgery will have a scar on the edge of the hairline

  • Hairline lowering cannot create a beautiful facial frame

The limitation of hair transplant

  • It may take a year to see the complete result
  • The operation time of a hair transplant takes many hours on local anesthesia, and they will feel uncomfortable during the operation
  • The number of hair grafts is not more than 4,000 grafts each time. It can reduce the forehead width by around 1.5-2 cm, depending on the hair graft density
